Medications in EOL SM

Medication Name Instruction
Fentanyl (use preferable with ESRF) 25 to 50mcg s/c hrly, PRN (For pain or SOB)
Morphine ( for opioid naïve patient ) – Higher dose may be needed if on regular opioids- seek advice 2.5 to 5mg s/c hrly, PRN (For pain or SOB)
Haloperidol 0.5 to 1mg s/c hrly, PRN (For agitation or nausea)
Midazolam 2.5mg s/c 2/24, PRN (For agitation or distress)
Glycopyrrolate 400mcg s/c 4/24, PRN (For respiratory secretions)
Hyoscine butylbromide 20mg s/c 4/24, PRN (For respiratory secretions)
