APNA Chronic Disease Management and Healthy Ageing Program – Darwin
Australian Primary Health Care Nurses Association (APNA) invites all nurses to attend the Chronic Disease Management and Healthy Ageing Program in Darwin.
Registering for this workshop is the first step in APNA’s Chronic Disease Management and Healthy Ageing Program (CDM & HA program). The highly engaging and interactive session will guide you through your role in empowering patients to manage chronic disease and healthy ageing, and best assisting patients to navigate the health care system and referral pathways effectively.
The program centres around four topics:
- Overview of Chronic Disease Management and Healthy Ageing (CDM and HA)
- Patient self-care management, Engagement and Communication
- Assessment, Care Planning and Care Coordination
- Support and Empowerment
The CDM & HA program has been designed to incorporate five key components to assist your learning: a one day workshop, self-assessment activity, online learning, a discussion forum, and a follow up with APNA.
Registering for the workshop is the first step, and you can tailor the other aspects of the program to work for you. The program runs for 12 months after you’ve registered for a workshop.
